FSG-GLOVE-03 — Glovebox glove drawn into rotating machinery

A glovebox glove becomes unstowed during high-speed fuel fabrication machining operation or spurious machine start-up, resulting in the glove becoming entangled in rotating machinery, tearing it away from its port, and causing an open glove port and breach of glovebox containment.
